Before You Schedule

To ensure smooth and timely patient intake, please email or fax all required patient information to referrals@occudoclive.com or fax 813-330-3332 when scheduling an appointment. Required information includes:

• Patient demographics

• 4600 Letter

• Medical records and claim status with accepted/delayed body parts (preferred)

Scheduling without complete patient information may result in delays in patient intake and appointment confirmation.

Important: Shortly after the appointment is scheduled, the patient will receive a text message with a required intake form. The form must be completed and submitted at least 24 hours prior to the scheduled appointment. Failure to complete the form by the deadline may result in the appointment being rescheduled.

This template helps request a confirmed list of Medical Provider Network (MPN) physicians who are available and willing to treat your client as the Primary Treating Physician. It is designed for use when the Medical Access Assistant (MAA) provides an MPN list containing physicians who are not accepting the referral.
